This grant aims to increase access to and improve the quality of behavioral health services within Federally Qualified Health Centers (FQHCs) located in The Rapides Foundation Service Area in Central Louisiana. It is designed to support the implementation of evidence-based models of integrated behavioral health, addressing both mental health and substance use conditions within primary care settings. The Implementation Grant is the second phase of a two-part funding opportunity, with participation in the earlier Assessment & Planning Grant required. Funding is intended to help FQHCs move toward fully integrated care, improve health outcomes, and address gaps identified during the assessment phase.
