Description
The New York State Office of Mental Health (OMH) funds six Teams to Promote Aging in Place in OMH Supportive Housing (TPAP), with at least two teams serving rural counties. This geriatric demonstration project supports residents at risk of losing community tenure because of unmet homecare, care coordination, service navigation, or social support needs. It serves adults aged 55 and older and younger residents with complex medical conditions requiring homecare.
Teams combine supportive housing, aging services, homecare, and paid peer support to develop person-centered plans reflecting residents’ priorities. Funding supports enhanced homecare, cross-system coordination, community connections, and individualized wraparound goods and services. Applicants may develop new programs or expand existing aging programs, provided they meet all program requirements.
