Description
This opportunity funds six interdisciplinary teams to support aging in place for residents of OMH (Office of Mental Health) Supportive Housing across New York State. At least two teams will operate in rural counties. The program targets older adults (55+) at risk of losing community tenure due to unmet home care, care coordination, or social needs, as well as residents under 55 with complex medical needs. Teams must be person-centered, holistic, and collaborative, involving case managers, homecare professionals, peer specialists, and local offices for aging. The initiative aims to reduce unnecessary institutionalization, enhance independence, and address social determinants of health and social isolation.
