The Minnesota Department of Health (MDH) supports community-based coalitions bringing together multisector partners and community members to strengthen mental wellbeing and prevent suicide. The program advances the 2027–2032 Minnesota Suicide Prevention Plan through locally driven approaches reflecting community strengths, cultures, needs, and priorities.
Funded work includes coalition development, community assessment, prioritization and strategy selection, action planning, implementation, evaluation, and sustainability planning. Coalitions use quantitative and qualitative data, community voice, and existing assets to select culturally relevant strategies that strengthen protective factors and reduce risk factors. The program emphasizes meaningful engagement of people with lived experience and communities disproportionately affected by suicide, comprehensive prevention approaches, and capacity to sustain effective partnerships and strategies beyond the grant period.
