    Description

The Rural Hospital Planning and Transition Grant supports small Minnesota hospitals working to preserve or enhance access to health services. Administered by the Minnesota Department of Health’s Office of Rural Health and Primary Care (ORHPC), the program funds hospital-led planning and implementation projects focused on access to care in rural communities.

Authorized by Minnesota Statutes, Section 144.147, the program provides two pathways: planning efforts and implementation of projects. Both are directed toward maintaining existing access to health services or improving that access. Funding is intended for eligible hospitals with 50 or fewer beds that meet the program’s nonprofit, hospital-type, and geographic requirements.
