--- ## Description The Hortense and Louis Rubin Community Health Fund provides grants to support programs aimed at promoting health and reducing the incidence and prevalence of chronic kidney disease and related risk factors—such as obesity, diabetes, and hypertension—within the Greater Capital Region of New York. Funding priorities include initiatives that encourage healthy habits and lifestyles, and efforts to strengthen health equity for vulnerable communities, including Black and Latino/x populations, urban and rural residents, and those facing barriers to accessing essential community needs.
