Description
This grant opportunity, offered by the Community Foundation for the Greater Capital Region, funds programs that promote healthy individuals, families, and communities by reducing the incidence and prevalence of chronic kidney disease and related risk factors such as obesity, diabetes, and hypertension. Special consideration is given to initiatives that strengthen health equity in vulnerable communities (including Black and Latino/x populations, urban and rural areas), improve access to healthy food and healthcare, and support home dialysis services.
