--- ## Description The Hortense and Louis Rubin Community Health Fund provides grants for programs that promote the health of individuals, families, and communities by reducing the incidence and prevalence of chronic kidney disease and related risk factors such as obesity, diabetes, and hypertension. The fund prioritizes projects in the Greater Capital Region of New York State and encourages initiatives that foster healthy habits, strengthen health equity (particularly for vulnerable communities), and improve access to essential community needs.
