Description
The Wisconsin Department of Health Services’ Division of Care and Treatment Services seeks providers to operate Projects for Assistance in Transition from Homelessness (PATH), a federally funded program supported by the Substance Abuse and Mental Health Services Administration. Its goal is to reduce or eliminate homelessness among adults with serious mental illness, including those with co-occurring substance use disorders, who are experiencing homelessness or at imminent risk of it.
Providers must prioritize street outreach and case management. PATH may also support screening, treatment, rehabilitation, staff training, referrals, and limited housing services. The program emphasizes connections to local housing organizations and services that can continue after a person leaves homelessness. Providers must prioritize eligible veterans and people experiencing chronic homelessness and integrate recovery principles into their services.
