Description
The Wisconsin Department of Health Services’ Division of Care and Treatment Services awards contracts to operate Projects for Assistance in Transition from Homelessness (PATH), a federal formula grant supported by SAMHSA. The program serves adults experiencing homelessness or at imminent risk of homelessness who have serious mental illness, including co-occurring substance use disorders. Street outreach and case management are required priorities.
PATH supports connections to sustainable mental health services, housing, and mainstream resources that remain available after homelessness ends. Additional eligible services address community needs not financially supported by mainstream services or behavioral health programs. Priorities include local collaboration, services for veterans and people experiencing chronic homelessness, recovery principles, and service areas with populations of at least 150,000.
