Description
This cooperative agreement aims to improve the overall health and wellness of American Indian and Alaska Native men by supporting a culturally responsive Public Health Nursing (PHN) case management and care navigation model. The program focuses on engaging men in preventive health services, increasing access to care, promoting early identification and management of health conditions, and supporting healthy lifestyle behaviors through coordinated, community-based interventions.
