The Rhode Island Department of Health’s Diabetes and Cardiovascular Health Program (RIDCH) offers short-term funding to connect patients with two CDC evidence-based lifestyle change programs: the National Diabetes Prevention Program, locally branded as Ready for Health, and the Healthy Heart Ambassador Blood Pressure Self-Monitoring Program (HHA-BPSM). The project supports screening, recruitment, outreach, and referrals through RIDCH’s secure online referral system.
The program aims to establish sustainable community-clinical linkages for chronic disease prevention and management. Participating organizations develop workflows to identify patients with prediabetes or elevated Type 2 diabetes risk and patients with unmanaged hypertension, then refer them for program enrollment. Populations of focus include adults, Medicare and Medicaid beneficiaries, African Americans, and Latinos.
