Description
Intermountain Health’s Social Drivers of Health Grants support programs and services addressing nutrition security or stable, quality housing through wraparound and support services. The Intermountain Community Care Foundation reviews applications for this funding. Both new and existing programs are eligible, including pilot initiatives that clearly address a social driver of health.
The grants focus on program and service delivery rather than construction or capital investment. Support can include limited small equipment and materials, with consideration for computer equipment when necessary and vital to project success. Funded organizations must report outcomes and are expected to track and share aggregated data about their projects and participants.
