Description
The New Jersey Department of Health’s Office of Minority and Multicultural Health (OMMH) offers this competitive grant to support one organization serving Black and African American communities in New Jersey. The work aims to reduce health inequities and barriers to care through tailored health navigation, outreach, education, and connections to state and community services.
The selected organization is expected to work directly with community members to assess needs, identify gaps in resources, convene partners, and shape its services accordingly. The program calls for a dedicated two-person team and a community assessment. Its first grant term runs from July 1, 2026, through June 30, 2027, with potential non-competitive renewal extensions.
