--- ## Description This grant opportunity supports nonprofit organizations working to foster healthier individuals, families, and communities by reducing the incidence and prevalence of chronic kidney disease and related risk factors—including obesity, diabetes, and hypertension—in the Greater Capital Region of New York State. The fund prioritizes initiatives that promote healthy habits and lifestyles, strengthen health equity (especially in vulnerable communities such as Black and Latino/x populations, urban and rural areas), and improve access to essential needs like housing, transportation, and food.
