    Description

    This grant opportunity funds initiatives that aim to improve community health by reducing the incidence and prevalence of chronic kidney disease (CKD) and related risk factors—such as obesity, diabetes, and hypertension—within the Greater Capital Region of New York State. The fund prioritizes programs that promote healthy habits (nutrition, physical activity, tobacco cessation, sleep, stress reduction) and address health equity, especially for vulnerable populations (including Black and Latino/x communities, urban and rural residents).
