The Tennessee Department of Disability and Aging (DDA) seeks qualified Innovation Coordination Agencies to serve the Medicaid Alternative Pathways to Independence (MAPs) program in Tennessee’s West, Middle, and East regions. Funded agencies support participants through person-centered planning, collaboration with providers, ongoing monitoring, milestone validation, and access to enabling technology.
The grant aims to expand service capacity, improve participant access and choice, and support anticipated program growth. Applicants may propose services in one or more regions. The work brings together planning, coordination, and follow-through to support MAPs participants, with awards made through a competitive application process.
