    Description

The Texas Health and Human Services Commission (HHSC) administers Family-Based Alternatives (FBA) to help children with disabilities living in institutions move into foster care, adoptive homes, or permanent family homes. The program serves Medicaid-eligible children younger than 22 with physical, intellectual, or developmental disabilities or medically fragile conditions. Its goal is lasting family relationships, with parents or legally authorized representatives participating in placement decisions.

The selected provider will identify children needing alternatives, recruit and train support families, match children with families, and ensure ongoing support. The project also includes permanency planning for children in nursing facilities, technical assistance to community-based providers, and statewide outreach. Family placements are intended as long-term arrangements, supported by individualized training, service coordination, and transition planning into adulthood.
