    Description

The West Virginia Rural Health Transformation Program (WV RHTP), administered by the West Virginia Department of Health, supports projects that expand rural care, strengthen the healthcare workforce, and modernize health services. Its announced awards support telehealth, mobile clinics, cancer screening, health information exchange, remote patient monitoring, and workplace-based care. Projects serve individual counties, multi-county regions, or the state as a whole.

The program organizes its work into five initiatives: Connected Care Grid, Health To Prosperity, HealthTech Appalachia, Mountain State Care Force, and SmartCare Catalyst. Together, these initiatives address access to care, employment support for people with chronic conditions or substance use disorders, health technology development, clinical career pathways, and provider productivity. Funded projects include rural recruitment, paid apprenticeships, shared care coordination, clinic technology upgrades, medical-supply drone delivery, and AI tools that reduce administrative work.
