Description
This opportunity aims to fund demonstration projects that establish coordinated Clinic and Community Collaboratives (CCCs) to integrate HIV care with broader women’s health services in areas of the Southern United States with disproportionately high HIV disease burden. The goal is to improve HIV-related outcomes, overall health, and quality of life for women living with HIV (WWH). Funded organizations will be expected to:
- Integrate HIV care with comprehensive women’s health services, including chronic disease screening, behavioral health, gynecologic care, menopause management, cancer screening, prenatal/postpartum care, aging-related care, and essential support services.
- Deliver culturally and linguistically responsive services.
- Participate in a learning collaborative to exchange best practices and lessons learned.
- Strengthen coordination among clinical, community, and public health partners for program sustainability.
Key outcomes include increased initiation of rapid ART, higher viral suppression rates, improved preventive health screening, better access to behavioral and aging-related services, and enhanced sustainability of integrated health service models for women living with HIV.
