Description
The Wisconsin Department of Health Services (DHS) supports local partner organizations developing the infrastructure and capacity to become Asthma-Safe Homes Program in-home asthma education providers. This opportunity targets Brown and Milwaukee counties through a Children’s Health Insurance Program health services initiative funded by the Centers for Medicare and Medicaid Services. It aims to improve asthma outcomes and reduce disparities among children ages 2–18 and pregnant individuals diagnosed with asthma and enrolled in Wisconsin BadgerCare Plus (Medicaid or CHIP).
Supported work includes staffing plans, operating procedures, training, community partner mapping, referral development, and client recruitment strategies. Grantees must also pilot in-home asthma education for up to 10 clients. Services combine asthma self-management education, home trigger assessments, and follow-up support, promoting appropriate medication and device use, asthma action plans, and coordination with health care providers.
