The Wisconsin Department of Health Services (DHS) seeks one organization to establish and operate a centralized Reentry Navigation Support and Training Assistance Center. Funded through the CDC Overdose Data to Action for States program, the Center combines statewide organizational capacity building with direct services for people leaving incarceration or navigating probation, parole, and extended supervision.
The Center will deploy reentry navigators to provide trauma-responsive case management, address supervision requirements, and connect individuals to substance use disorder treatment, recovery support, and essential community resources. It will also provide training, education, and technical assistance to organizations serving justice-involved people. Priorities include correctional facility partnerships, pre-release communication, warm handoffs to care, overdose prevention tools, and sustained recovery support, with the goal of reducing fatal and nonfatal overdoses.
