    Description

The Wisconsin Department of Health Services (DHS) seeks an organization to operate the Southern Region Children’s Resource Center, funded through the Title V Maternal and Child Health Services block grant. The center will support families and professionals serving children and youth from birth through age 21 who have, or are at risk for, chronic physical, developmental, behavioral, or emotional conditions requiring services beyond those generally needed by children.

As part of a statewide network of five regional centers, the organization will provide free, confidential resource connection services by phone and email. Services include helping families navigate complex medical diagnoses, educational and community resources, specialized provider networks, and health insurance options. The center will also help families build peer networks and identify advocacy opportunities.
