Description
The New Jersey Department of Health’s WISEWOMAN program funds evidence-based strategies to prevent and manage cardiovascular disease among women ages 35–64 who are at or below 250% of the Federal Poverty Level, are underinsured or uninsured, and are enrolled in the National Breast and Cervical Cancer Early Detection Program.
Funded work addresses cardiovascular disease and its risk factors through prevention and self-management strategies. The program also supports system-level, bi-directional electronic referrals; implementation or expansion of team-based care; and efforts to strengthen the cardiovascular disease workforce’s capacity to address social determinants of health.
