Description
This forecasted opportunity from the Indian Health Service (IHS) aims to support aftercare programs for American Indian and Alaska Native (AI/AN) youth who are discharged from Youth Regional Treatment Centers (YRTCs). The cooperative agreement is designed to enhance culturally appropriate, community-based aftercare services by improving care coordination, fostering partnerships across the continuum of care, and building YRTC capacity to address barriers, support effective case management, and improve long-term outcomes for youth.
